403(b) Information

403(b) Tax Sheltered Annuity Plan (TSA)

As a benefit to its employees, Astoria School District sponsors a supplemental, tax-advantaged, retirement savings program authorized by Section 403(b) of the Internal Revenue Code. Such plans are often called 403(b) Plans or Tax Sheltered Annuity Plans or simply TSA Plans. Online Access and Enrollment

New and Current Participants

  • Employees and former employees of Astoria may create an Online Account to monitor participation activities associated with the Astoria 403(b) Plan.

  • CCC encourages you to establish an Online Account, which will allow you to review information about your participation in the Astoria 403(b) Plan, including contributions to various vendors, account balances (if provided to CCC by vendors), and various transactions processed on your behalf by CCC (loans, hardship distributions, contract exchanges, plan-to-plan transfers, rollovers, required minimum distributions, etc.).

  • Click Here to create an Online Account.
  • Current employees who have created Online Accounts are encouraged to use CCC’s online enrollment system to submit Salary Reduction Agreements (SRA)* and manage contributions while logged in.

  • However, if you do not wish to create an Online Account but wish to enroll, you may submit an SRA in paper format. Click Here to access a printable SRA form.
*Please note: You must have an established account with a vendor in the plan to begin contributions.

Roth Contributions Available

Astoria allows employees to make Roth 403(b) contributions to any Astoria vendor that will accept such contributions. Roth contributions are deferred from paychecks to investment accounts on an after-tax basis, as opposed to Traditional 403(b) which are deferred on a pre-tax basis.

Please note: Roth 403(b) contributions can only be made to designated Roth 403(b) account. You may not send Roth contributions to an existing Traditional 403(b) (pre-tax) account. Roth 403(b) accounts are available from select vendors, please review the information listed on the Astoria 403(b) Vendors page for vendors that show "403(b) Roth Account Available" after the vendor name where applicable, then follow the Enrollment Steps to begin participation.

CCC's Relationship with Astoria

CCC serves as the third party administrator (TPA) of the 403(b) Tax-Sheltered Annuity Plan (TSA) for Astoria on a contractual basis, which began on 8/22/2014. Astoria maintains a Selected Vendor 403(b) Plan Document that may have provisions that differ from the majority of CCC Full Service Clients and may restrict the addition of new vendors and/or exchanges or transfers to vendors other than those expressly stated in the Plan Document. CCC is the primary point of contact for any questions concerning the Astoria 403(b) Plan, please contact us with any questions.

Obtaining Transaction Authorization


CCC offers Plan Administrative Authorizations for plans CCC administers in various formats, depending on the transaction type.

CCC offers Authorization Vouchers for the following transaction types:

  • 403(b) Distributions to the Participant (for qualifying events of Severance from employment and attainment of age 59.5)
  • 403(b) Rollover Distributions (for qualifying events of Severance from employment and attainment of age 59.5)
  • 403(b) Contract Exchanges within the Plan
  • 403(b) Plan-to-Plan Transfers out of the Plan

For all other transaction types

Participants may request plan authorization by completing the following steps:

  1. Complete a CCC Transaction Submission Cover Sheet and submit it, along with your completed vendor forms, to CCC. You may submit these documents to CCC using one of the following methods*:

    • You may securely upload the documents to CCC by clicking here (please include instructions on where CCC is to send the authorization),

    • You may fax the document to CCC at 503-968-7802, or

    • You may mail the document to CCC at the following address:

  2. Once CCC receives your transaction submission we will review the documents, verify eligibility for the transaction.
  3. If appropriate, CCC will provide plan administrator's authorization for the transaction.

The CCC Transaction Submission Form is to be used to submit any transaction type except 403(b) Loans and 403(b) Hardships to CCC.

*Please note: some Vendors impose specific requirements before they will process certain (or any) transactions. Before you begin the process of submitting transaction, be sure to check for any special Vendor requirements (such as original document via mail only) by visiting their “General Information” page(s) available on the List of All Vendors page or contacting your vendor directly.
