If Vanguard holds a Payroll Slot in your Employer's 403(b) Plan and you would like to apply for a Vanguard account, either download, complete, and submit the "Account Application Form/Packet" linked to under the "Vendor Forms" heading in the right-hand column of this page or contact CCC and provide your Social Security Number and email address. If you take the latter approach, CCC will submit your information to Vanguard and Vanguard will send a partially completed Account Application Form to your email address.
